If they format data, they're going to need to adb push the file in the step before flashing. You should also specifiy that's only flashing the rom and has nothing to do with firmware or radios. Also you mentioned to make a nandroid backup in step 1. But your step 3 would erase that nandroid backup so I'm thinking you need to add the step of moving the nandroid backup files to a different storage device or don't have them format the data partition.
If I was new enough to need these directions and made a nandroid backup in step 1 and followed the directions and realized that I had formatted data which erased the zip I was supposed to flash as well as my nandroid, I'd be quite pissed, but worse I would be frustrated and defeated, and I would hate to see that drive away some users.

you are simply flashing the stock 1.15.605.4 rom... which is kinda pointless TBH

I thought that "format data" didn't touch the virtual "sd card". It just wipes the apps and app data. But there is another option to wipe the storage. When I installed a ROM I chose in TWRP to format data, cache, and davlik from their wipe menu. It did not touch the Downloads folder I had saved the ROM to.
Factory reset in recovery is what you are thinking of, which is equal to "format data excluding /media"
In the factory reset menu it says something like "wipe data -/media, cache, and dalvic cache"
If you go to advanced wipe, and wipe the data partition instead of using the factory reset selection it wipes the entire partition, doesnt exclude /media which is the location for /sdcard

A full wipe is wiping all of those, however it is not always needed depending on whether or not you actually use Apps 2 SD etc
Most of the time you only need to wipe option 1 (with the occasional 2 as well)
2 is included by 1, for a full wipe do 1 (and 3 if you have an ext partition on your sdcard).
I've been flashing custom roms and updates for a about a month now, with once every 3 days on average. Wipe option 2 never finished successfully, and showed a message that I had to use adb to perform it or something. (not sure of the message though).
Anyway, out of no where, it worked today for the first time. I was on aHero v0.3 Rom.
What could be the reason? Is it the Rom? The only thing different I've done in this Rom was use Root Explorer.
this is a know bug in the recovery image. it tries to wipe dalvik-cache on the ext partition, which ofcourse fails if you don't have one. nevermind, the wipe works regardless of the error message.

I did partition my sd card recently but never actually made use of it. (unless some rom used it automatically) .. anyway that should explain it.

I never get an error when I wiped mine and I don't have an ext partition

I've only ever used option 1. No issues to report here.
does Wipe SD literally deletes the content of the SD card? like formatting?

no. it is called "wipe SD:ext" because it wipes only the ext partition of the sdcard. not sure what it does exactly, but i am pretty sure it just deletes the "app" and "app-private" folders on the ext partition, i have other files there that are not deleted when i wipe it.

OK, if I understand it properly I have to lose all my apps and data to install or update a new ROM? Of course I may use Titanium backup but that is quite long process to restore all apss and data again, right?
Yes, mostly you have to wipe all. Sometimes it says that you only need to wipe cache/ dalvik cache if coming from a previous version. Do you have the free version of Titanium Backup? If you do I would suggest downloading either the paid version or MyBackup Root (it's free). Then you get one click restore. It still takes a lot of time, but you can do other stuff while it's doing it
Oh, yes, it is a good feature to restore on one click. I thought it is weird. No I understand when it is just in paid version. One more thing. Once I was restoring apps, I used app2sd script but TB restored somehow in the wrong place because the notification about low space appeared. Even though I did not get it before when I used the script. How to make sure thet TB restore apps to the right place? Thanks!!

OK, so for the RCMixHD v3.6 with Data 2 Ext(4) they say a
"Full wipe is required"
What does that mean? It is totally unclear and not being explained anywhere in the ROM tutorials. Pretty weird, because it seems rather important.
I have the ClockworkMod Recovery bootware on my HTC Desire, so how does one perform a "full wipe" without also wiping HBOOT and such (when it is S-OFF I would understand a "full wipe" actually wipes *everything* on the internal NAND/SD) ?

It's strange how both of these are rarely really required, while I STILL see this "full wipe required" being advertised for most roms. It's nonsense. Why would you need to wipe your entire SD? If a certain formatting would be required for the rom to function properly, it should include a script that does that for you and prompt before wiping your SD-card.
Also, the Factory reset is only required in very rare cases.

Well if I am updating from the same rom I will do cache and dalvik only.

Every time I flash a new ROM or restore a nandroid backup, I do the following:
1. Wipe Dalvik cache
2. Wipe data/factory reset
The second step formats /data, /cache, /sd-ext, and /sdcard/.android_secure, so that's a pretty thorough wipe if you ask me. I wipe the Dalvik cache as well to be as thorough as possible.
Optionally, you can format /system under mounts and storage in CWM recovery.
I am currently using mik_os 6.5(http://forum.xda-developers.com/showthread.php?t=946354) and Clockwork Mod recovery. I tried to install 6.5.2 on top of it (wiping cache and dalvik cache) but it gets stuck on the lg logo. After going through the first post, I even formatted /system but that didn't help either.
Finally, I flashed 6.5 and it is working fine. Any pointers would be helpful.
1) mounts and storage/ wipe cache

4) wipe data or factory reset
5) install zip from sd card

Boot into clockwork recovery.
go to backup and restore -create a backup of your current rom (Note:If already backup has been done then avoid this step, backup created using amonra or thunderg wont support restoring using clockwork)
wipe data/factory reset
wipe cache partition
mount and storage-format system
mount and storage-mount usb storage (make sure usb debugging is enabled)
copy the rom zip file to the root of your sd card
unmount
install zip from sd card-choose the rom zip and install
flashing takes 3-5 min
reboot!
just remeber to backup all ur app using titanium and restore after a fresh install and whnever u update no need to wipe anything just flash over the old version until unless mention by the dev
I didn't want to do a factory reset, however; I still went for it after doing a nandroid backup.
I followed all the steps mentioned by sshbz, but it still got stuck at the boot logo (the one with website address written).
Finally, thanks to the Nandroid backup, I reverted back to 6.5 without having to go through titanium restore.
For 6.5.2, the rom link is given in post 1 and mik_os had also posted a fix for clockword mod users.
I have tried the below
1. Install only the rom
2. Install rom and then the fix
Neither worked. I think I would have to wait for the next update now unless someone can make this work for me.

i wipe data factory reset.. then wipe cash..then dalvik cash..then flash rom.. been doing that for a year wit no problems
I dont know for sure as I to am new to this device. But Id think it would be the same as any other device
I do know for google wallet you need to clear before flashing or it borks on this device,,,

Factory reset wipes /data and /cache
after Factory reset, wipe System and you should be good to go on a new or fresh installation from zip. i dont think wiping boot is necessary but you can. so do

Dalvik-cache is in /data so you dont have to wipe that.

Sd-ext is or was the best way to move apps to the sd card. But with the new OS it isn't required if the app is made right

I factory reset twice, wipe dalvik twice and wipe system twice. I don't wipe cache since the factory reset does it.
Now that it was clarified dalvik is wiped with the factory reset I'll probably stop doing that. I just wish it clarified it since it specifically indicates the cache was wiped.

I did a factory reset today and flashed the stock 4.1.1 Polish ROM via Odin. This process seems to have wiped my files (including photos etc) from my S3 (internal memory, not SD card). I've flashed plenty of ROMS before and this has never happened.
Anyone know if it's possible to get them back? I guess I've installed new apps etc now so a lot of the data would have been overwritten. Frustrating!

Unless you backed up on PC or sd card...
Factory reset in S3 will always wipe internal memory as it shares internal memory for applications.
If you want to restore try some data recovery software that supports SD card.

That would be the only way. There is a very small window to recover data wiped from flash memory and on a phone it would be even more difficult. Maybe a Clockwork backup?
Internal SDcard is mounted on the system partition via Fuse..so if you format the /system partition..sdcard is formatted as well..
Pointers for future..
Use CWM for flashing ROMS..(I hate Odin )
Backup your internal SDcard on your PC regularly
Perform a Nandroid Backup on your ExtSDcard before flashing any ROM.
If you ever need to factory reset..use the wipe data/factory reset option in CWM and not in Stock Recovery
For a clean install, if you want to wipe system partition..use wipe /system option under mount and format in CWM..it just wipes the system files and your SDcard data will be safe..
You will not face such issues if you in the future if you follow these pointers.

I've recently rooted my phone, installed new ROMs/Kernels. Prior to installing the ROM, I wiped my storage, my cache, my dalvik cache, etc. However, when I was looking around my folders, I noticed files from pre-wipe - in other words, it didnt really wipe my phone out. Now obviously I dont want dead applications taking up space - how can I properly wipe the phone before installing a new ROM?

Try to do a factory wiped from the phone. I think that does the trick.

you can also use this tool. I guess it is the same as revolutionHD from above member posted whereas you have different options/stages of wiping the phone.
You can either use super or mega wipe (leaves you without anything on the phone!! so be careful)

As what I have read, it wipes whatever a full wipe will and should not wipe nandroid backups.. However I have not used it before and it's better to wait for confirmation from someone who used it.

I can confirm it doesn't wipe the efs partition - as that could well and truly screw up your phone, especially if you didn't have a backup stores somewhere.
Superwipe wipes your data, cache, preloaded and system partitions - pretty much your entire internal sd card, if I'm not mistaken. I really like the megawipe tool that's been mentioned here too. You should give that a try when next updating.
As to the backups - I've no idea. It all depends on where the backups are created. I think CWM backups are created in data partition which would mean that if you did super wipe they'd also be deleted.
I never back up though so I can't be 100% sure as I've never needed it.
