Stuck In Boot Loop - Boot Failure on LG Page at Start Up - Verizon LG G3

I have gone through other forum entries that were similar and did not see anything exactly the same. Came back to G3 after 6 months since I had the common issue of screen freeze/flicker and fading out. I did paper trick where folded paper goes over camera port on motherboard to get it at least temporarily functional. That did the trick as far as screen working for now.
My major problem began when I tried utilizing this technique I found in forum in hope of a more permanent solution:
I spent days of googling and there are only temporary solutions for this problem.
After testing my phone for 2 days in different conditions I realised that the screen is going crazy when the light is changing too much.
If you do a sensor calibration everything will go back to normal:
-enter in Hidden Menu (model dependent)
-select Device Test
-select MT
-OK to restart the device
-OK and do not touch the screen for few seconds
-go to Individual Items+Power (you must push power button when you select this item)
-select Sensor Test and take care to not block the proximity sensor when you make the selection
I got as far as MT in device test and phone restarted. However, it won't get past the first LG screen. It hangs there for a few seconds and the words "Boot Failure" appear. Is there anything I can do to get out of this boot loop. I tried hard reset with volume down and power and get same result. I also tried hooking into PC using volume up for download mode and get nowhere with that either. Is there anything else I can try to fix this problem?
I greatly appreciate any help anyone can provide. I did notice the last resort debrick method by R-T-B but it seems a little complicated for me and I was hoping there might be an easier solution.

When you tried to put the phone in download mode, did you use the original cable, and use it with a USB 2.0 port that comes directly off of your PC's motherboard, not USB 3.x and boy through a USB hub?
If so, what happened exactly?

Yes, I used original cable and made sure it was USB 2.0 port. I hold volume up button and plug other end of USB cable into phone and it goes right to the black screen with just LG logo and Life's Good. Directly under the logo in white font is Boot Failure. I can't seem to get past it. Phone will shut down by itself and automatically turn back on going to same boot failure page.
One thing I forgot to mention in original post. I had not used phone in a few months and forgot I had taken SD card out. So when I did the MT in device test SD card was not there. Also, phone is rooted. I had a Jasmine ROM 9.0 or 9.1 (35B). I have put back SD card into phone which I am not sure matters or not but get same result.

SD card shouldn't matter for anything.
What does it say in Windows' Device Manager in the ports section when you attempt to put it into Download mode? LGE... or QHSUSB_BULK 9008 mode?

It shows LGE Mobile USB Serial Port. When I enable show hidden devices it shows 2 LGE items under Ports. One has COM4 and the other COM41.
Also, USB 2.0 connection to PC is directly to port on back of PC. Not connected through hub or anything. Also, although I don't know if it matters but I had TWRP flashed as well.
Its frustrating that I can't get phone to do anything but reboot back to LG screen. I don't understand why just going into Hidden Menu got phone stuck in never ending boot loop. I could understand if I flashed something wrong but I didn't do anything like that. At this point I don't care about any files or anything on phone, just want to get it in working condition.

There are a lot of dangerous things in the hidden menu, that's why it's hidden.
If you continue to not be able to get it into Download mode, look at the thread in the General section for forcing it into QHSUSB BULK 9008 mode, and then there are different recovery instructions that still needs the 10B TOT.
Or @R-T-B might be able to fix it for you. I don't know what the deal is with two LGE devices showing, bit I've never taken any steps to show hidden devices.
Won't hurt to try flashing the 10B TOT even though there phone doesn't look like it's in Download mode. I don't know if the existing hidden port 41 LGE device would be sufficient or if you should change to port 5 one to 41. Normally you have to change the port 5 one, but both are worth a try.
Of course, it the phone constantly reboots too quickly to try flashing, then ignore that advice.
Sounds stupid I know but try anything like leaving the SIM card and Micro SD card out?

FYI apparently it seems normal that enabling hidden devices in Device Manager would show the Port 41 LGE entry. I flashed the 10B TOT to my wife's phone and after it booted and I enabled hidden devices, I could see the "new" valid entry on Port 5 and the old hidden entry on Port 41, so it's no matter, and if you choose to change the Port 5 one to Port 41, just proceed even though it warns you there's another device on that Port. A reboot of your PC might fix it too.

Did you ever get this sorted out? My g3 same thing. Hidden menu, device test, and then reboot but stuck at lg logo screen. I’m unable to bring up mini os for the hidden menu to escape the device test action. It just stays on with the lg logo on the screen.
Thanks for helping.

Is my phone bricked?! (grey battery screen)

Hi guys,
I figured I'll post again here since I think I bricked my phone.... I was trying to install Eugene's Vibrant6.zip and I got stuck on the Vibrant Logo with Samsung at the bottom. I tried to reset with both volume buttons and power button and got to grey battery and nothing else...
What I did prior to this was I formatted system and data on the recovery screen as per some posts here and it would just boot into a loop. Windows won't recognize a device and it just shows grey battery image and it doesn't seem to be charging. Anyone can help me?
EDIT:
Out and in the battery, plugged in to USB and it just loads the battery image and it's all grey color as if fully discharged and just stuck on it. Device is also not found in my system so I can't do any adb commands. If I just reset it without the cable it would just sit on the Samsung boot loop. Anything I can do?
UPDATE: Ok, some good news! I heard the sound as it connected via USB, I can see the 2 drives, but they read as Removable Disk and I can't access them...
adb reboot recovery or download
error: device not found
What if simply format the drives via Windows right click menu? Will that do any good or bad? What are my options here? Any help will be appreciated!
UPDATE: Well... looks like this is pretty much a dead phone.... I right clicked and selected Format in My Computer and it just says There is no disk in Driver F: Insert a disk and try again.
Can I a mount an image somehow via MagicISO? Or is there something simpler that I just can't figure out?
Again, thanks for your help!
Thanks.
Has anyone experienced this or am I the only one?

Sorry I have never heard of this or witnessed it while flashing anything to my device. Obviously the battery screen signifies that the phone is off but is in charging mode and hitting the power button will boot the device up. (I'm sure you already know of this). Never heard of it being stuck at that screen after flashing. Hopefully you can get this solved soon. Good luck

Slow down for a second. Your phone is definitely not bricked. Ive been over the edge, and been able to bring my phone back to life.
To clarify, you flashed vibrant6.zip through clockwork mod, and then the phone got stuck on the samsung S? How long did you let it sit there?
It sounds like just your ROM is messed up.
Are you able to "get into recovery"? Holding both volume buttons while you power on the phone, without it being plugged into usb.
Do you have Odin? And have you ever used it?
Its totally frustrating when your phone gets messed up and you don't know what to do.
I flashed the stock ROM via Odin and then wanted to install Vibrant6.zip. For that I entered into a recovery mode via Clockwork and as someone in the other thread advised, I formatted system and also data, don't recall exactly. Then it looped to a standard recovery mode and I just rebooted the phone. This is where it got stuck... Vibrant in the center and Samsung at the bottom of the screen. To test it I left it like that from night till this morning. In the morning I woke up it was on the same boot screen.
Before that at night, after seeing it hand at the boot, I held both volume keys and power, waited for it to reboot, and immediately it showed a circular progress bar, but not moving just highlighted the 12 o'clock bar and second later I see the battery image all in gray and nothing else. It just sits on that grey static image without actually charging it (no green section or flashing dots)
Boot logo only appears if I am trying to reset without the USB cable. On the boot loop I plug in the cable and I hear the USB connecting it, I see the 2 Removable disks, but both inaccessible and adb commands don't recognize the device.
Very weird, as in turned off mode it's not charging at all.. just shows this grey battery image with a circle progress froze on 12 o'clock bar.
That's pretty much it.... I was never able to enter into recovery nor download via the buttons. It would just always reboot into this grey battery screen....
Completely helpless.... I did order a new battery from TMO, maybe the battery is fried and that's why it's not charging and just sits on this stupid static grey battery screen.

Holding both volume keys and releasing power once screen goes blank, only show Vibrant logo again... I have tried this a million times
With USB cable it shows grey battery screen, without the cable just a Vibrant boot loop...
I'm assuming that you have the Samsung drivers installed.
Try the following steps:
Take out the battery and then put it back in but don't turn the phone on.
Plug it via USB.. does anything show up on the screen?

Yes, I have the drivers installed as I was able to locate my device and use adb commands to enter into recovery and download. Also Kies worked.
Doing what you asked simply load the grey battery image and nothing else with a progress bar not spinning. Just sort of hangs on this image and that's all.
Booting without the USB cable loads the Vibrant boot loop. Connecting USB at this boot loop state, would play a sound as if it connected to my windows, but I can only see Removable disks that are inaccessible and adb doesn't find the device...
Very strange.... trying to use the button keys to enter into recovery or download is no use, as that just won't ever happen, even when phone was working fine.
Any other suggestions? I would return the phone for replacement... but I am not sure how it would look with all the files still on the internal disk including root if I am not mistaken... I had only few hours of sleep because of this headache

Are you sure that you're doing the right things to get to download? I've been in this situation and I wasn't able to get into recovery but I was able to get back into download mode.
I believe if you return it back and they find out it was rooted, they will charge you for it. If not, it'll appear on your bill.
I'm sure that you're doing the wrong things to get into download mode. If something is appearing on your screen it is a soft-brick and you're not bricked.
1. Take out battery, put it back in. (to turn the phone fully off)
2. Don't turn it the phone on!
3. Plug the phone into your PC, do not hit the power button!
4. See if the phone shows anything on the screen (powers on its own when plugged in)
IF the phone screen does 'turn on' after being plugged in without you hitting the power button, so far so good.
5. Now while it's on like this, hold the volume up and volume down buttons.
6. Hold the power button as well as the volume buttons until the screen goes black.
7. As soon as the screen goes black, let go of the power button! But DON'T let go of the volume buttons!

A tip to get into download mode is to put the phone HORIZONTALLY (sideways) and it will make it a lot easier.
I am pretty much positive about it. I know a lot of people can easily enter into recovery or download modes, but I know many can't and I am one of them. It would just load the grey battery image instantly as I let go of the power button.
1. Take out battery, put it back in. (to turn the phone fully off) DONE
2. Don't turn it the phone on! DONE
3. Plug the phone into your PC, do not hit the power button! DONE
4. See if the phone shows anything on the screen (powers on its own when plugged in) FIRST THING THAT APPEARS IS A CIRCULAR PROGRESS BAR THAT CHANGES TO A GREY BATTERY IMAGE WITH A BAR IN THE MIDDLE OF IT. STATIC IMAGE, NOTHING ELSE.
IF the phone screen does 'turn on' after being plugged in without you hitting the power button, so far so good.
5. Now while it's on like this, hold the volume up and volume down buttons. DONE
6. Hold the power button as well as the volume buttons until the screen goes black. DONE
7. As soon as the screen goes black, let go of the power button! But DON'T let go of the volume buttons! DONE
THE STUPID GREY COLOR BATTERY IMAGE APPEARS AS BEFORE....
I'm thinking maybe battery is fried and that's why the grey color screen appears and not the usual screen where you see the battery charging...
After reading this thread it's clear that my handset is one of the new ones that have recovery/download modes disabled via the volume+power buttons.
So that pretty much covers it... I hope TMO will not ***** about the files that are on the internal sd that I am unable to access and remove... unless anyone else has any fresh ideals how to gain access to the disk
I think I might have a similar problem.
I accidentally deleted the touchwiz framework so I can't do anything ON the phone. It boots up, and gets me to the lock screen. As soon as I unlock, I am greeted with dozens of unending force closes.
I can get into both download and recovery mode, however, I have not been able to get ADB to recognize the device. I am attempting to push the stock ROM to the internal memory to flash over everything.
I'm not sure what to do to get ADB to work but it's driving me nuts!
Stuck at vibrant? I have a fix

Feels like I'm having same issues..
the previous post requires opening the phone and I don't have tools for that..
Here's my issue.
I can't get into Recovery or Download and computer won't see phone to Odin.
I've got drivers installed and AIO Toolbox as well as ADB(which won't work because of computer not seeing phone.
Using the manual option(vol+ vol- power), nothing works.
I've done all the tricks like unplug it then hold vol keys plug it insert battery..
When I do plug in the phone to to even just get a charge in it goes the the battery, but my is is frozen or something. doesn't turn green or have a charging animation. After about a minute or so it was start up and boot loop. I read something about a bad battery? I've got two of them and they seemed fine before..
Before all this I had to Odin and go to stock. I was attempting to get ics back but I was being lazy and trying to do as little as possible..bad idea. I was following slim ics install and flashed cm9 daily, slimics and commons..and now this..
I'm not sure what to do..I usually search around xda or google..10 hours later.. idk..Anyone able to give help please?

Try asking another question. But I suggest using a USB jig. They usually work. Then odin
Sent from Mount Olympus via Hermes
Did you ever get it fixed man? I literally did the very same to mine a while back and It works like butter as we speak. It is fixable with a mini-usb jig or dongle. You can make your own with about $3 worth of parts from radio shack but I recommend buying one, it will be easier. I for the record made mine simply twisting together the resistors and though it was tough to touch the right pins it did eventually work and get me back into download mode.
Hopefully you already got it fixed though. If not I can help you out.

[Q] Can't get to Download Mode - stuck on loading screen

Hi All,
So, I tried to flash Cyanogen mod last night using the instructions here: http://wiki.cyanogenmod.com/wiki/Samsung_Vibrant:_Full_Update_Guide. I was coming from a BIONIX ROM that flashed fine.
Anyways, the phone went into a reboot loop in the recovery screen and I foolishly just held the power button until it stopped. When I turned it back on it was broken. That is, now all it does is display the white Vibrant logo in the middle of the screen with the smaller Samsung logo below it. If I hold the power button for about 8 seconds the screen goes black for about 2 seconds and then comes right back up to where it was. I can't seem to even turn the phone off any more. It just stays on that screen permanently unless I hold the power button, in which case the screen is off for 2 out of every 10 seconds. Yanking the battery of course turns it off, but putting it back in immediately returns it to the state I've described.
I've scoured these forums and tried every button press + battery state + cable state + SD card/SIM state combo suggested, but the phone won't go to download or recovery mode. I found one thread describing the exact same problem. The OP said he'd try a jig and then disappeared without any further updates. I'm going to try to get my friend to make a jig using one of these sets of instructions
http://forum.xda-developers.com/showthread.php?t=819551
http://factoidz.com/how-to-unbrick-your-samsung-vibrant-galaxy-s/
but until then my questions is: does anyone have any experience with this problem (can't even turn the phone off)? If so, do you have any recommendations? Did a Jig work?
keep trying to get to download mode, pull battery, plug usb in to computer and to the phone, while holding both volume up and volume down pop in the battery. People have tried hundreds of times with it not working and then all of a sudden *bam* download mode.
So... my friend made me a jig and it works in that I can see the shoveling droid indicating download mode. However Heimdall/Odin won't recognize my phone. Heimdall's zadig utility won't locate the phone either, even with the cable plugged in. I'm, once again, stumped.
Just an update; I've tried Odin on three different computers, two Windows 7 (64 bit) and a Mac (with Heimdall). None of them seem to recognize the phone. Device manager on the Windows 7 machine does not update when I plug the phone in (in Download mode via the jig or not) and I can't find anything about Samsung in it anywhere. However I have installed the Samsung drivers; a few different sets I've found around the forums. I've also tried a couple utilities like the Vibrant toolbox and one click unbrick. Nothing seems to have any effect. I was so hopeful when the jig worked and got me to download mode, but now it's seeming I'm actually bricked. Has anyone seen a phone that could get to download mode but still be bricked? Oh, and I checked the cable using a friend's phone; her Mac recognized her phone using the cable. Haven't tried her phone on my Window's machines though.
Once again, any help is more than appreciated.
Have you tried to see if adb is working? If it does then try adb reboot recovery? If it works there is a file I can post for you later (is in my pc) that can restore your sbl.bin and fix your download mode problems.
Thanks for your help everyone, the problem was apparently resolved by a new cable; which is strange because the old one works for data transfers... just not for flashing apaprently. :S

[Q] Phone won't turn on??

I'm currently using a P769 model with JB, and it's never been rooted. I remember the phone was 100% charged when I looked at it three days ago and I didn't use it at all the day after that, but when I tried to use it yesterday it didn't turn on and it hasn't been responding since (I did try to charge it a few times). Actually the exact same thing happened a few months ago but it fixed itself in a few hours, but this time it's been two days and I hope it doesn't mean my phone is completely bricked The main thing is that I want to avoid doing a factory reset if at all possible since I made the mistake of not creating a backup.
I've tried some recovery tools (LG Mobile Support along with third-party) but the problem is that nothing seems to recognize my phone as connected. Weirdly enough, when I plug it in it without the battery, it connects and disconnects continuously (the device manager refreshes itself every 3-4 seconds); I've updated the USB drivers and everything, but nothing seems to work. Not sure what the Recovery Mode key combination is but I'm assuming the phone has to boot first for Recovery Mode to even be accessible anyway, so that's probably out of the question. However I have tried Vol+, power and Vol+, power, home, and nothing happens.
To make the recovery software recognize your phone, you must put it in S/W mode by disconnecting your phone, taking out the battery, reinserting the battery and then, while holding the volume+ button, plugging it into your computer(keep holding volume+ until the screen says S/W just to be safe). At least that's what I always did.
Hmm... I tried that, and nothing happens on my phone, but I get a popup from the system tray on my computer that says "USB device not recognized: The last USB device you connected to this computer malfunctioned, and Windows does not recognize it" which appears and disappears repeatedly. It appears that no software can recognize the phone as connected at this point. I think it might be a random battery failure but I don't know how to tell D:
The thing is all I really need out of the phone is the contact and app data if possible, which is why I'm so hesitant to do a hard reset :| I read around and I'm assuming what I should do is to flash the bootloader but I'm not sure how to go about doing that since I'm getting zero response from the phone itself.....
UPDATE: Tried a hard reset anyway, but still nothing from the phone. I attempted fastboot, however, I don't know how to make it work since "OMAP4430" does appear under TI Devices in Device Manager plus an "Unknown USB [device failed enumeration something something]" under USB Controllers, but that's it.... nothing appears under Other Devices, which seems to be where the bricked phone should show up. adb doesn't seem to recognize it either.
i have the same problem with my lg p760.
any solutions?
you had a solution for this problem ?

[Q] LG Access LTE (L31L) Stuck on boot screen and wont go into recovery mode

Hey guys im new to this thread and a website said to see if you guys could help me. Well basically I tried to make my music louder on my android and it didnt like that. Now it wont boot or go into recovery and to make things worse, i made it go into charge only mode a long time ago. Since i cant open it or view files since its on charge only mode. I think im screwed. Is there any way i can fix this? Thanks a lot for anything. I tried to hard reset and all of that but it goes into a black screen then just goes back to the LG loading screen. It basically just loops itself everytime i try to enter recovery. It gets stuck on the tracfone page. This is a straight talk phone. Its the LG Access LTE L31L. Ive looked everywhere and havent found anything that works and thought that you guys would hopefully help me. Is it just a paperweight or is it fixable? Thanks!
In the computer the phone isent reconized as anything??
I found one in a car I just bought, has the lock for PIN enabled and cannot enter phone.
Tried all the reset options found on web, nothing brings me to recovery, searched the forums here for this model phone, nothing found.
Tried Volume Down + Power, released Power when LG logo appeared for 3 seconds then pressed and held, it would reboot sometime long sometimes short, but would not enter recovery, tried both volumes up and down, volume down and home button, every combination I could find...
I had a SIM card for a tracfone that was active and wanted to see if this phone would accept it...
Any other tricks available? Computer dings that it sees the phone, but since I cannot get past PIN screen I cannot choose what function USB should be and nothing is seen in Windows Explorer.
Any LG master files to force open the phone through the PC when locked like this or ???
Sorry to hijack or open an older thread, but not much action here for this model phone...

Did I brick my phone?

I was in the process of installing a new rom on my phone, I have been rooted for months, everything was fine, now when I installed the new rom via TWRP and rebooted my phone wont turn on, I just get a flashing notification light alternating between blue and green and it just sits there, I can't even boot back into recovery using Volume down + power, I just get a black screen.

so it seems like my phone is working in the background, but the screen is still black, if I tap things I can hear noises, I think I hit the emergency call button and was hearing the phone being dialed.
So now I need to find out why my screen would just go out like that, and if its the brightness that is completely low or something else.
if I plug in my phone to my computer and pull out the battery, my screen turns on with a flashing missing battery box and I can see the background of my phone, but I can't do anything else.
This kind of problem is commonly caused by a mismatch in partitions from various versions. Am example would be if you flash the 35B bootstack and then after that flash either JasmineROM 9.0+9.1 or SkyDragon 6.02, and you don't reflash the 35B bootstack again after that.
Both of those ROMs have an incomplete collection of partitions from 24B which would overwrite the ones from the 35B bootstack, and the mix of the 24B ones with what 35B ones that weren't overwritten results in that.
If you go through the motions of putting the phone in Download mode (you can pull the battery to make sure and stay with it off), and if you already have the Verizon LG drivers installed, you can see the phone is in Download mode by looking in Device Manager in the Modem section.
Use LG Flash Tool 2014 to flash the 10B KDZ, re-root with the Stump app, put TWRP on with TWRP Manager from the Play Store, then reflash what you want.
If you want the 35B bootstack on either of those two ROMs, flash it after the ROM, not before. If you don't want the 35B bootstack, flash the 24B bootstack before flashing the ROM.
I can't get the phone to go into download mode, with the battery off, the second I plug into my PC the phone turns on instantly into a faded setup wizard that looks like it has 2 wizards over laid on each other and in the middle a dark cloud with a battery icon and flashing red question mark.
holding volume up and power won't do anything.
Also my pc is being stubborn, I installed the LG g3 Verizon drivers and it shows exclamation marks on the phone's entry in device manager, should I do this in windows XP?
so it seems my phone does go into download mode, the overlay I am seeing is the setup wizard for the phone and the firmware update screen behind it..
still can't get the drivers to fire up though

After having installed the Verizon LG drivers, I recommend rebooting the PC (with the phone disconnected).
The correct way to get into Download mode is by, with the phone completely off, press and hold the Volume Up button (but not the power button), then while continuing to hold the Volume Up button, connect the original USB cable to a USB 2.0 port directly on the PC, not through a hub) and to the phone. In a moment you should see the Download mode screen and you should let go of the button.
The first time you do this on a different USB 2.0 port, Windows can take up to 10 minutes to load the driver that you had installed. Leave the phone connected for at least that long and then verify if the icon in the Modem section of Device Manager looks normal then, not with the exclamation point.
Edit: I didn't see until later that you say it does go into Download mode - never saw the Download mode overlaid by the phone setup wizard. If that's accurate, something's weird since Download mode doesn't get loaded in Android.
I think this phone is shot, I followed your instructions, the phone boots up to the firmware upgrade screen but I can still see "setup wizard" behind it really faint, and the PC continues to not recognize the phone

Very, very strange.
You can't get into TWRP either? Hold Volume Down and while holding it, press and hold Power, then when stock recovery pops up, let go of the buttons, and use the volume and power buttons to select Wipe Cache or Factory Reset - either should take you to TWRP.
What ROM did you install, anyway?
If you can get into TWRP, try flashing the 35B bootstack from @xdabbeb 's VS985 v2.0 ROM thread in Devleopment, then try to get into Download mode again. You might even be able to select from TWRP's reboot menu off the main menu, to reboot into Bootloader, which I think takes you to Download mode too. I haven't actually tried using it that way too - I'd try the regular way of getting into Downloade mode first.
I just did the same thing to a send LG G3 I bought, you gotta be kidding me....
Tried to install Jasmine 9.0, then updated to 9.1 and ran the debloat option, that seems to be killing my phones, this was a $150 lesson that I am avoid Jasmine Roms from now on. I can restore the new phone either, won't boot up into recovery, or anything.
my PC picks up the phone, but the screen is black so I can't tell if its in download mode, and LGflashtool 2014 won't recognize the phone.
I am at a loss for words.

I forgot to mention in the JasmineROM thread - I think that was you talking about the issue there too, right? Even if the phone screen is black as long as you see it in Device Manager under the modem section, then you're in Download mode. Then you can use LG Flash Tool 2014. Make sure and give Windows up to 10 minutes to load the driver. Don't use a USB hub.
Pull the battery to make sure the phone is off. Connect your original USB cable to a USB 2.0 port on your Windows PC - don't have the phone end connected. Press and hold the Volume Up button, and while keeping it pressed, insert the USB cable into the phone. Even if it doesn't vibrate - that's okay, although it might, too. Let go of the Volume Up button after two seconds. After giving Windows enough time to load the driver, you should see it in the Modem section. If you don't see it, check under the Ports section and see what it says.
So when I plug my phone into my PC my device manger shows LGE Mobile for VZW USB Composite Device and it says this device is working properly, but when I try to run LG flash 2014, I get an error saying my Device isn't plugged in
well I got it past the "not connect" part but now I am getting a connection to server failed, try again in a moment

Just ignore error dialogs like "can't connect to server". Drag the error dialog out of the way, don't click any buttons in it. Within 5-10 minutes you should find your phone successfully flashed.

completely ignore it? because I hit "OK" and then on the screen screen my S/W upgraded starts and errors out at 4%
Ok so I noticed the progress bar behind the connection error box, but that is the same one that errors out at 4%. The only thing I can try now is the original cable which I don't know where it is, I've tried 3 different usb cables already though.
Use The Specific Trademark Program Of Your Device On your Computer, E.,g(Flashtool,Odin,) And Flash An Stock Rom On It

Yes, completely ignore it. Don't hit "OK" or any other button on the dialog, just drag the dialog out of your view if you must. It's common, it happens a lot although not all the time.

*Error Update* So with this leaked LP image, I've been playing with kdz'ing quite a bit and with a new Windows install I got to experience a few of the issues being reported by others that I have never seen, along with trying to troubleshoot some others. Did I manage to fix them easily you ask?? Hell yeah!
USB Disconnected Issue: This is a driver issue. Very easy fix, just download the latest Verizon drivers from the link I posted above and install them without phone plugged in. Power off phone, hold up, plug into usb and WAIT for download mode drivers to install (as stated several times in this thread). Click the balloon to watch the progress, this took about 10min on an i5/ssd machine - be patient. This is the predominant issue I think people are having. The drivers while in download mode are:
-USB Composite Device
-LGE Android phone or LGE AndroidNet for VZW USB Serial Port COM5 (this is the important one)
-CDC Serial or LGE AndroidNet for VZW USB Modem (this will fail if you are on a rom with certain VZW items removed - not a problem, its not needed)
-CDC ECM LGE AndroidNet for VZW NDIS Ethernet Adapter
If driver install fails, manually update them by pointing windows to the driver file you downloaded. I am not going any further here, use Google.
??????'s In The Field: easy, don't change region or language from Korean as stated in the OP. Leave it alone, it will flash in English I promise you. Result = all fields readable in English while running
Connection To Server Failed: Simple, ignore the window. Just drag it away, but don't close it. This is an offline hack originally indented to run online, deal with the window!
Progress Bar: The tool will initially analyze the phone, then when flashing starts, pay attention to the phone screen which will now say COM5. The blue progress bar on the phone is the important one. At around 80% on the tool screen, the phone will reboot. The flash is over and the bar will continue to progress into the 99% while rebooting - then the program will crash. You can disconnect now, you've been successful.

This part is confusing me, I am getting an error updating, but what does he mean by the latest 35B leaked image? should I try this with 10B?

23A, 23B and 23C_00 were leaks of Lollipop. Try what exactly?
I was mainly trying to highlight:

Im still trying to get around the issue when it gets to the S/W update and then errors out around 4%.
I don't know what else I could be doing wrong.

Try any/all of the following?
Reboot the PC?
Use a different USB 2.0 port?
Use a different quality USB cable?
So you're saying even if you ignore the error dialog that it stops at 4%?
Yes 4% and I get a complete error.
I am going to try an official LG cable to see if it helps

Since I said two days ago about using the original USB cable and you didn't comment, I figured you were already.
