I just want to know the name of this widget or launcher theme (not sure) which shapes icons like this picture .

either its mclock. you can get this effect by using the xml file. follow the link for lots n lots of xml files for mclock.

It looks very much like Pandahome with a specific theme applied which so far I haven't been able to find.

EDIT: After further research I think it is probably Make Your Clock Widget which is available free on Android Market

it's laggy sometime you add a widget on the empty screen
just dont let the desktop too empty,fill it with any widget

I guess this screen uses go launcher that has a tons of themes and icon packs where you can find the square ones.
About the widget you can use uccw and pick ehatever you'd like including the one @ your attached pic
